• I would perform medication management using telehealth. I feel this would provide more access to care. In the Baltimore area, there is a shortage of medication management providers and it is often months before you are able to get in to see a provider. Other specialties can perform therapy but only APRNs, Physicians, and PA’s can prescribe medications, therefor this is a more needed service.
The type equipment you would use (RPM, Store & Forward, mHealth, Videoconferencing, or a combination):
• I would use videoconferencing as this allows me to see and speak to the patient concurrently. It is important to be able to visualize the patient to assess for non-verbal cues and also for medication side effects or allergic reactions.
The type population/healthcare issue you would address:
• My ideal population is pediatrics. I feel it is important to intervene with this population early as they will ultimately be our future leaders. From my experience in the Psych ER and with my own children, it appears mental health has taken a toll on the children. I have also personally experienced the effects of the provider shortage when seeking help for my own child.
Your role in the program:
• My role in the program would be a PMHNP medication management provider.
What you would evaluate about the program:
• I would evaluate equipment, HIPPA compliance, patient education, and reimbursement rates.
Legal considerations you would address:
• I would address HIPPA compliance, Licensing compliance, and also how the practice addresses having a yearly face to face.
Other professions that you would like to involve:
• I would like to also involve IT, social workers, psychotherapist, psychologist, and clinical nurse specialists.
Protocols you would develop:
• I would develop protocols for documentation, state/board compliance, HIPPA, how to utilize the equipment, proper Telehealth etiquette, face-to-face requirements, pharmacological therapies and non-pharmacological therapies.
How would you Consent the patient:
• I would consent the patient prior to the visit in a new patient packet as well as consenting the patient verbally prior to each session. -
