    In this Telehealth encounter, I – Anna Przybylska- was a nurse practitioner and Andres Peralta was a patient who was discharged a week ago from the hospital after being treated for a stroke. The patient’s consent for Telehealth visit was obtained at the beginning of the meeting. Next, I have asked Andy about his symptoms past week with medication reconciliation and allergy history, along with a short physical examination. Most of the time was spent on patient education about the importance of taking medication every day, which will eventually prevent the next stroke, as well as addressing his recent right sided weakness in hand and leg, to prevent falling. For that reason, I would add to the treatment plan occupational therapist (OT), physical therapist (PT), and speech therapist (ST). To make sure that Andy is taking his Crestor medication every day we discussed a Medisafe app. To make sure that Crestor is working and for cholesterol level control, I have added to the treatment plan a labs order in 6 months. At the end of the visit, we recap one more time, what I will email to Andy: referrals to PT, OT, SP, Medisafe link and lipids panel. Also, we agreed to meet for f/u in a month and see how the PT, OT and ST is helping him with getting his muscle strength back. I received great feedback from Andy about a telehealth visit. He found it very helpful, especially in his right sided weakness condition, he doesn’t have to worry about driving to the office.
