Arlene De Los ReyesParticipantAs a future Family Nurse Practitioner, I would like to be involved in a telehealth program that combines videoconferencing and remote patient monitoring (RPM) to manage chronic conditions in underserved adult populations, especially those with hypertension and diabetes. This blended model would support real-time virtual visits for assessment, education, and treatment planning, while RPM tools such as Bluetooth-enabled blood pressure monitors and glucometers would allow daily health data to be securely shared. Videoconferencing would help foster a personal connection to improve trust and adherence, while RPM would support early detection of complications. In this program, my role would include conducting virtual visits, interpreting patient data trends, tailoring care plans, and collaborating with other healthcare providers.
To evaluate the program, I would track patient outcomes including A1C control, blood pressure regulation, and reductions in hospital visits. Legal considerations would include using HIPAA-compliant platforms, maintaining accurate documentation, and ensuring I am licensed to provide care in the patient’s state if needed.
