I was the provider during a developmental evaluation of a 6 month old baby who had spent some time in the NICU prior to discharging home. Sara was my patient who was the mother of the baby and had some intermittent financial and postpartum challenges.
I evaluated Kim as a provider for a developmental evaluation of a 6 month old baby who was in the NICU and now had come home.

    Coaching comments:

Kim did a great job hitting everything on the telehealth checklist. She began the appointment by introducing herself, confirming consent and checking all the equipment. Throughout the appointment Kim was engaging, empathetic and educational. Most of all she was professional. She asked open ended questions to keep the patient engaged during the appointment, and was actively listening to the patient’s responses. I appreciated that she made a point to inquire about the mother’s mental and physical health. She also was considerate of the baby just getting down to sleep when asking about an assessment.

Please provide your general feedback on using the checklist here.
The checklist was a helpful tool to make sure that every telehealth appointment is productive for both the patient and provider. It is easy to forget the fundamentals of conducting a thorough appointment, and this checklist creates accountability for the provider and security for the patient. I will use this checklist in my future telehealth experiences as a provider.
