DARYA BELYAYEVAParticipantFor this videoconference I was the diabetes case provider for teaching and follow up care. This patient will benefit from ongoing care. Other professionals that could have been involved include nutritionist, diabetes educator, and pharmacist. Telehealth equipment that could be helpful for the patient would be health apps (mHealth) to help with carbs counting and increased physical activity as well as BG monitoring.

DARYA BELYAYEVAParticipantTelehealth Model you would select and why (Store & forward, videoconferencing, RPM, mHEALTH, a combination): Videoconferencing with specialties, including cardiology, urology, neurology, psych, pain management, podiatry and dermatology.
The type equipment you would use (RPM, Store & Forward, mHealth, Videoconferencing, or a combination): GoToMeeting for videoconferencing with HIPAA-compliant desktop, mobile, and phone conferencing features. I will use security options such as one-time passwords and integrate scheduling and chat apps
The type population/healthcare issue you would address: Geriatric population in long-term care setting. Many of the patients in the long-term facilities are wheelchair-bound or bedbound, and transportation is often an issue. When residents unable to complete follow up or new appointments with specialties due to wait time or transportation issues, it leads to conditions exacerbations, ED visits, and hospital readmissions. The residents will benefit from the availability of virtual appointments with specialists for many health problems, improving wait times, transportation cost and risks for missed appointments.
Your role in the program: I will continue to serve as a primary care provider to the population in the facility and coordinate the videoconferencing program.
What you would evaluate about the program: Transportation cost, ED visits and hospital readmissions rates. Patient’s satisfaction.
Legal considerations you would address: Consents, HIPAA, privacy due to residents sharing rooms
Other professions that you would like to involve: Specialties as listed above
Protocols you would develop: Referrals to specialties and emergency protocols
How would you Consent the patient: Residents will sign the consent for participation in telehealth with specialties upon admission to the facility. There will be a general consent for all specialties in the facility. Other consents might be required by the participating specialties and will be signed by residents or responsible parties prior to the first visit. -
