    1. Identified the reason for the visit.
    2. Confirmed consent with the patient and explained telehealth.
    3. Obtained a history of the situation.
    4. Asked about treatments used prior to the visit.
    5. Provided telehealth education using an appropriate learning style.

    Today I met with Brittany who has just gotten discharged from the hospital due to a stroke. She is having some right side weakness and is having slower speech. She is not ready to drive or leave the house yet, so telehealth was a great option for her. She did not have another person with her, so the exam was more brief than usual. We discussed important safety concerns and when to go to the emergency department. Other professionals that could be involved with this patient are physical therapy and speech therapy. Technology that can be used is mobile health. The patient could check her blood pressure with a cuff that syncs with her phone and the phone could share to our clinic to monitor her blood pressures.
