    Assisting with weight loss goals via telehealth
    Telehealth Model – videoconferencing. This would allow a collaborative effort for providers to meet with a patient to discuss weight loss goals and medication management to assist with these goals.
    Equipment: Store and Forward would still be needed for labs, but videoconferencing would be ideal for the rest of the management. I would also use digital scales, BP cuffs, and digital food diaries to monitor and assist with management for weight loss.
    Population: The type of population this would address is the morbidly obese, who would like to start medication-driven weight loss therapy.
    FNP Role: I would identify those that are interested in medication-driven weight loss therapy and conduct the initial interview, as well as manage the follow-up telehealth visits. I would also be the prescribing member for these medications.
    Evaluation: I would evaluate the process of medications working the best with the least amount of side effects, and if the combination of medication, lifestyle change, and at-home monitoring of food and weight has a great effect on weight loss than traditional store and forward programs.
    Legal: I would have to address HIPPA and any legal contract issues from compatible food diary apps and scales to monitor weight.
    Other professions: I would involve a pharmacist and a dietician, and maybe include an endocrinologist for those that have comorbidities of diabetes.
    Protocols: I would have to develop protocols for medication selection, program eligibility, and patient safety; as well as, follow-up protocols and monitoring parameters.
    Consent: I would consent the patient at the initial store and forward clinic visit for the treatment and management through telehealth videoconferencing in the future.

    Madison was the provider for the first case of a patient with a new diagnosis of type 2 diabetes. She was well organized and conducted the pace of the telehealth appointment well. She guided me as the patient through my diagnosis and plan and addressed my concerns about having to use insulin. She addressed my health needs of eye exams, foot exams, and daily BG monitoring and educated me on my medication. She also involved a referral to a dietician to help me address my gas station food problems.
    She could have placed a referral for podiatry or endocrinology but don’t think they were needed just yet.
    Another technology to use would be a digital food diary that could help the provider point out areas of lifestyle modification to help with BG control.
