    In this scenario I was the health practitioner for a diabetes assessment and Tonya Vermette was the patient. This scenario was a chronic case management clinical telehealth visit for a DMII patient. The assessment and health history were a focused exam on the DM management. There was the ability to reference recent lab work and also to engage the patient in sharing her blood sugar monitoring records. I also was also to simulate the use of a home BP and HR device which is a low-tech option for a chronic disease management program to provide patients for home daily use as well as for chronic case management tele health. Diabetes Management is one of many chronic disease management models that tele health could be leveraged to improve access and improve the quality of care. I was able to explore further with the patient her disease management questions as well as explore gaps in her overall health care for a referral for nutritional assessment and eye care referral. I found this assignment to be helpful in integrated the learning objectives of telehelath implementation.

    Evaluation- Tonya did a great job in her clinical cardiopulmonary assessment. As a practitioner Tonya had a clinical strong bed side manner that translated into the telehealth call seamlessly. She achieved a professional rapport with me as a patient and was able to provide a high-level care delivery experience for the patient in an equal manner to what could be achieved in a in person visit. She was able to effectively communicate and establish the goals of the encounter and also assure that the patient had clear next steps in the clinical care – labs, follow-up, escalation of care, and ongoing communication. Tonya was highly effective in meeting the objectives of this assignment and demonstrates competency.
