1) Telehealth Model
The Telehealth model that I would select is synchronous video conferencing. I picked this model because I believe that live interaction is the best option to provide a high-quality patient experience. Additionally, this model can help reduce stress and commute time for the patient/provider while providing the same services as an in-person examination.2) Equipment
The type of equipment needed for synchronous video conferencing include a computer or tablet with a microphone and camera, internet connection with a router and Wi-Fi, peripherals (not required), and a secure videoconferencing software such as Zoom.3) Population/Healthcare Issue
The type of population I would treat include pediatric and adult athletes in a variety of settings such as high school, college, recreational/club, and professional. Additionally, the healthcare issue that I would address would be a broad range of injuries within my scope of practice. However, this will include but surely isn’t limited to injuries of the musculoskeletal system, nervous system, circulatory system, respiratory system, integumentary system, digestive system, reproductive system, lymphatic system, and endocrine system.4) Role in the Program
The role I would play in the program is to provide effective, convenient, and low-cost healthcare for my patients. Additionally, I would also play a role in the program for collecting data, tracking metrics, and obtaining patient feedback to promote quality improvement.5) Program Evaluation
I would evaluate the program using the SPROUT measurement framework to address the following domains: health outcomes, value of healthcare delivery, patient/provider experience, along with program characteristics, performance and implementation.6) Legal Considerations
The legal considerations I would address include reviewing the Public Health Emergency (PHE) waiver proposed by the DEA, following my state practice act, reviewing the proper billing/coding/modifiers guidelines for reimbursement based on the type of insurance and the parity laws for private payers, ensuring the program is HIPAA compliant and aligns with the Office for Civil Rights (OCR), and establishing a Business Associate Agreement (BAA) with the vendor.7) Professional Involvement
The other progressions I would like to involve include physicians (orthopedic and speciality), nurses, physical therapists, chiropractors, dietitians/nutritionists, psychiatrist, and mental health counselors.8) Protocols
The protocols I would develop will be based on my scope of practice, the state practice act, and any specific policies/procedures needed to adequately provide optimal healthcare for my patient population. However, some potential protocols I may develop for the athletic population include emergency action plans, policies for emergent medical situations such as cardiac, spine, and heat/cold illnesses, along with a referral protocol.9) Patient Consent
I would obtain patient consent following the states policies because informed consent varies state by state. The patient consent can be either written or verbal and typically consents are valid for one year. -
