    I was the provider for a 6 month-old’s gastric tube follow-up appointment. Baby Madison was discharged home from the NICU 1 month ago with a gastric tube, and we were conducting a telehealth visit to see how her and her mother were doing with the g-tube. Another specialty that could be utilized for this visit would be speech language pathology. A patient portal could also be used to provide additional resources and reminders.

    I would like to start a telehealth program that focuses on NICU follow-up for families discharged from the NICU that may not live close by or have easy access to our in-person follow-up clinic. I would use video conferencing so that parents/caregivers are able to communicate with any smart device or computer. As stated above, this would serve the patients recently discharged from the NICU. My role in the program would be as an NNP, assessing the patient, conversing with the parents about how they feel the transition home has been going, and providing any follow-up education/referrals that may be needed. I would evaluate the rates of readmission, additional education topics, and parental satisfaction. Legally, I would follow all national and state laws regarding telehealth programs including HIPAA, consent from legal guardians since the patients are minors, billing, and the storage of patient information. I would also involve the therapies that were consulted during the NICU stay (OT, PT, speech therapy). If possible/necessary, I would also try to include any consults from other physicians (i.e. surgery, neurology, cardiology, etc.). I would include protocols regarding billing, escalation of care, documentation, and scheduling since some patients may not be eligible for these visits. I would obtain consent with each visit, also following all state laws regarding the matter.
