    Meant to add this to my original post. The case that I was the provider for was “Stroke”. The patient was recently discharged to home and still experiencing weakness with concerns for safety at home. Other technology that could be used is RPM to gather objective data such as vital signs. Other professions that should be involved in this case are :social work, PT/OT, Speech, and home nursing.

    Telehealth model: This telehealth project would be through live interaction (videoconference) in combination with store and forward. Ideally these telehealth models would be set up simultaneously to use for our lower level NICUs in the health system to consult expert services at our level IV NICU. Live conferencing can be used in real time with a patient or in an emergency, and store and forward can be used to forward relevant imaging or diagnostics to the NICU team for analysis.

    Equipment; majority of this telehealth model is rooted in mhealth: all current providers have iphones that can send and receive imaging quickly. Addition of larger screens such as ipads or mobile computers would be beneficial

    Type/Population healthcare issue: this would increase access to expert clinical advice for lower level NICUs or hospitals that have an ill neonate without the proper knowledge or resources to care for the infant.

    Evaluate: Patient outcomes, number of transfers

    Legal: HIPAA considerations for mobile devices, out of state consult practice authority. Issue of identification of billable services, issues with proper consent obtainment in emergency situation

    CNS Role: A potential consulting service for other NICUs. Education to staff and outreach.

    Other professions: Physicians, NNP, PA, Respiratory therapy

    Protocols: Emergency telehealth protocol including on-call list for telehealth service, consulting protocol, billing protocol.
