Telehealth Model & equipment: Combination of Store & forward (management of asynchronous needs) & videoconferencing (synchronous visits0 since both are needed for the management of patients.
The type population/healthcare issue you would address: Address the WCC needs of rural pediatric patients.
Your role in the program: Provider and Administrator
What you would evaluate about the program: Monitor the demographics of the population served, residential location, diagnoses, and clinical diagnoses
Legal considerations you would address: minimal reimbursement for the provision of primary care services.
Other professions that you would like to involve: multidisciplinary: MD, LISW, ect
Protocols you would develop: determine which WCC would be safe to complete by route of video conferencing and develop protocols to conduct these sessions
How would you Consent the patient: Annually in clinic
