Telehealth model you would select and why. Telehealth visits can be an invaluable tool to any primary care or internal medicine practice to follow up on a host of chronic or minor acute conditions; patients that have transportation or mobility issues or live in rural areas could benefit tremendously from the telehealth model.
The type of equipment that I would select is videoconferencing; most people now have access to a smartphone, tablet, or computer, or they have a family member with access to these devices.
The type of population/healthcare issue you would address: Adults with both chronic and mild acute issues could easily be managed via telehealth.
Your role in the program: Provider
Legal considerations you would address: ensuring patient privacy, confidentiality and obtaining patient consent to conduct a telehealth visit.
Other professionals that you would like to involve: other providers, specialists, PT, OT, or social workers.
Protocols you would develop: I would have clear protocols outlining what conditions can be safely managed via telehealth and what conditions/symptoms would require a face-to-face visit.

Passcode: *RM=Ef+6In the second scenario, Tanya was recovering from a recent CVA, which resulted in right-sided weakness. Amy provided a post-discharge visit via telehealth. Amy used therapeutic communication effectively to alleviate Tanya’s concerns and expressed empathy throughout the visit. Amy’s tone of voice was very reassuring and calming. Amy’s environment was secure, and there were no distractions during the visit. The provider made a referral to speech therapy and also provided education on home bp monitoring and reviewed symptoms that should prompt the patient to seek emergency treatment. She also encouraged the patient to inquire about setting up her upcoming neurology appointment via the telehealth platform.
