    If I were to develop a telehealth program as a Family Nurse Practitioner, I would focus on caring for patients in low socioeconomic areas using a combination of videoconferencing, remote patient monitoring, and mobile health. This approach would help improve access to care while still allowing me to build strong relationships with patients and monitor their health between visits. I would focus on vulnerable populations who may not have transportation, lack of insurance, or limited access to healthcare. Many of these patients have chronic conditions like diabetes or hypertension, which can be better managed with consistent monitoring and support. With telehealth, they can have the resources to be able to follow-up as needed without having to worry about other burdens. As an FNP, I would provide virtual assessments, manage medications, and focus heavily on patient education, especially around lifestyle changes and disease management. I would also help connect patients with community resources.
