    I was in DPT group 34, I was the provider for the stroke patient and the patient for the ankle injury case. I conducted a thorough initial evaluation of a patient with right-sided weakness after suffering a stroke and being discharged from the hospital. I was able to look at the patient’s limitations and what they are able and not able to do functionally. I was able to gather history on the patient’s environment and their limitations in their activities of daily living. I provided patient education on safety and bed mobility. I recommended the patient try to come in for an in-person visit if they are able to further assess their balance and strength deficits. I also recommended the patient see an Occupational and Speech therapist as he could benefit from their services to further address his functional and speech limitations. To make this telehealth encounter more successful, I feel that the patient should have another person to help guard them so that I can better assess their balance in standing or even a better-equipped environment so the patient is safer.

    My partner, Frank Tinker, provided a thorough initial evaluation on an ankle injury with the utmost respect and care for the patient.
