Telehealth model: I would select a combination of videoconferencing, RPM and mHealth for my telehealth model. The videoconferencing would allow me to assess symptoms and conduct visits while seeing my patients. The RPM can be used to assess pertinent vital signs like pulse oximetry from the patient’s home. The mHealth can be helpful in sending patients information and reminding them of their appointments.
Population: The target population for my program would be children and adolescents with transportation issues with chronic conditions such as asthma.
Role: My role in the program as a Pediatric Nurse Practitioner would be the provider role. I will conduct assessments, provide education, determine plan of care, and refer to other disciplines as needed.
Outcomes: I would evaluate the patient outcomes in my program such as their satisfaction, follow-up rates, and adherence to treatment.
Legal considerations: HIPPA is the main legal consideration I would like to address because telehealth can pose a risk to patient privacy if it is not conducted properly.
Other professions: This would depend on the chronic condition that the patient is dealing with. For example, if the patient has asthma, I would consider involving pulmonology. Especially if it is poorly controlled.
Protocols: I would create a protocol that sets the frequency of visits and medication adjustment guidelines. I would also create a protocol of what to do if the asthma treatment fails.
Consent: I would obtain consent by sending it electronically via a secure platform prior to the visit. This consent would explain the purpose, benefit, risk, privacy protections, and alternatives to the telehealth visit.
