    As a nurse executive, my focus would be on assessing the best technology and tools to fit my clients. Currently, virtual nurse models where patients are sent home from the hospital and will still need nursing interventions before official discharge are being explored. This program involves videoconferencing as well as store and forward information. Nurses will visit the patients virtually 3 times weekly with 2 home visits. The patient population will be patients who have been assessed and determined to be eligible based on specific criteria such as ability to follow directions and ability to communicate through video conferencing. Other criteria focuses on patient diagnosis and chronic condition. The goal for this program is to keep high risk patients from being readmitted to the hospital. My role would be to ensure that the people, process, and technology are put in place for a successful program. This program would require and interdisciplinary approach with nursing communicating with the patient and care provider throughout the patient’s participation in virtual home care. Evaluation would be reduced readmits as well as patient, family, and clinician satisfaction. Consent would be given prior to discharge from the hospital. Protocols would need to be developed to outline the criteria for participation as well as reassessments to determine if the patient is still a candidate for home health.
